目的 探讨贵州省黔南和黔东南地区少数民族肥胖指标与胆结石病的剂量反应关系,方法 基于“中国多民族队列研究”的横断面调查数据,选取贵州省黔南、黔东南地区布依族、侗族、苗族30-79岁的成年人17,084例。使用Logistic回归模型分析肥胖指标与胆结石病的相关性,并使用限制性三次样条模型分析其与胆结石病的剂量-反应关系。结果 黔南和黔东南地区少数民族胆结石病总体患病率为9.0%,女性(9.6%)高于男性(7.5%)。胆结石组和非胆结石组在性别、年龄、种族、吸烟史、高血压、高脂血症、BMI、LAP、VAI、WHTR、WHHR、WC、体重、TG、HDL-C和GLU方面存在统计学意义差异(P<0.001)。与非胆结石组相比,胆结石组在年龄、体重、TG、GLU、BMI、VAI、LAP、WHTR、WHHR 和 WC 方面的值更高。在以胆结石病为因变量的逻辑回归模型中调整混杂因素后,发现BMI、WC、WHtR、LAP和WHR具有统计学意义。随着这些肥胖指标的增加,患胆结石疾病的风险也随之增加。进一步分析其剂量-反应关系中的关联强度,发现随着BMI(P非线性=0.0046)、LAP(P非线性=0.0004)、WC(P非线性=0.0422)和WHR(P非线性=0.021)水平的增加,其与胆结石病的关联强度呈非线性上升趋势。随着WHtR水平(P 非线性=0.2107)的增加,其与胆结石病的相关性呈线性上升趋势。结论: 在贵州黔南和黔东南地区少数民族中,肥胖指标与胆结石病呈线性和非线性剂量反应关系。倡导居民加强肥胖控制和预防是降低胆结石病发病率的重要手段。
