I certify that I am a citizen of the United states by birth. I was born at Los Angeles on 1896 . . .Camera OK, [stamp and signature of] Donald C. Nokes, 1st L[ieutenan]t Ord[nance] Dep[artmen]t, Signed: 'Dick' Whittington" -- carbon copy of War Department page. The negative envelope indicates that the Whittington studio fulfilled one print order.
